Overview
Thyroid management encompasses the clinical strategies used to evaluate, treat, and follow disorders of the thyroid gland, spanning functional, structural, and malignant disease. For functional disorders, management centers on restoring euthyroidism—levothyroxine replacement in hypothyroidism, and thionamides, radioiodine, or surgery in hyperthyroidism—guided by serial TSH and free thyroid hormone measurement. For thyroid nodules and cancer, management integrates risk-stratified diagnosis (ultrasonography, fine-needle aspiration, and increasingly molecular testing) with definitive treatment. In differentiated thyroid carcinoma this typically combines surgery with radioactive iodine ablation and TSH-suppressive thyroxine therapy, whereas medullary carcinoma requires adequate preoperative evaluation, surgical clearance of nodal disease, and, in advanced cases, RET-directed systemic agents. Image-guided ablation offers a less invasive option for selected tumors and recurrences, and radiation safety considerations apply after radioiodine therapy. Long-term outcome studies and large institutional series inform follow-up intervals, recurrence surveillance using thyroglobulin or calcitonin, and the balance between overtreatment and adequate control. Rare thyroid malignancies, including leiomyosarcoma and metastatic deposits, demand individualized approaches.
