Overview

Insomnia is the most prevalent sleep disorder, defined by persistent difficulty initiating or maintaining sleep, or by non-restorative sleep, despite adequate opportunity, accompanied by clinically significant daytime impairment such as fatigue, mood disturbance, and reduced cognitive function. It may be a primary disorder or arise secondary to medical, psychiatric, pharmacologic, or lifestyle factors, and chronic insomnia is associated with increased risk of cardiometabolic and mental health conditions. First-line management emphasizes cognitive-behavioral therapy for insomnia, and research in this area examines factors influencing the effectiveness of brief, workshop-based delivery of such therapy, alongside non-pharmacological and device-based approaches including peripheral somatosensory stimulation, pulsed magnetic therapy, comprehensive music therapy, and physical activity. Mechanistic and contextual work addresses the endocannabinoid system in circadian and sleep regulation, delayed sleep phase syndrome across cultural and genetic backgrounds, and the relationship of sleep disturbance to conditions such as hip fracture, night eating syndrome, and narcolepsy. Objective measurement of sleep quality, including actigraphy, supports both assessment and evaluation of intervention. The journal publishes peer-reviewed clinical trials, interventional studies, and reviews addressing the assessment, behavioral and physiological treatment, and comorbid associations of insomnia and disturbed sleep.
