Overview
Anxiety disorders are a group of psychiatric conditions characterized by excessive, persistent fear and worry together with physiological arousal that exceeds the demands of the situation and impairs functioning. The category includes generalized anxiety disorder, panic disorder, specific phobias, and social anxiety disorder, each defined by the focus and pattern of the anxious response. Their neurobiology centers on hyperactivity within fear circuitry, particularly the amygdala and its connections, modulated by neuropeptides such as cholecystokinin and by dysregulation of the hypothalamic-pituitary-adrenal axis and its cortisol output. These mechanisms link the subjective experience of fear to measurable autonomic and endocrine changes. Anxiety frequently emerges or intensifies during sensitive periods and in the context of other conditions: perinatal and postpartum presentations, early-life pain, and comorbidity with depression, trauma, and chronic medical illness are well recognized. Treatment is multimodal, combining psychotherapy, especially cognitive-behavioral approaches, with pharmacotherapy and with physiological self-regulation methods such as biofeedback and heart-rate-variability training. This journal publishes peer-reviewed research in this area, including studies of the neurobiology of fear and the role of cholecystokinin, perinatal and medically comorbid anxiety, and interventions ranging from psychological therapy to biofeedback and complementary techniques aimed at reducing anxiety and arousal.
