Overview
Acute coronary syndromes (ACS) are a spectrum of conditions caused by an abrupt reduction or interruption of blood flow to the myocardium, most often when an atherosclerotic plaque in a coronary artery ruptures or erodes and triggers thrombus formation. The resulting ischemia, and in many cases myocardial necrosis, defines a clinical continuum that includes unstable angina, non-ST-elevation myocardial infarction (NSTEMI), and ST-elevation myocardial infarction (STEMI), distinguished by electrocardiographic changes and by the release of cardiac biomarkers such as troponin. Patients typically present with chest pain or discomfort, which may radiate, accompanied by dyspnea, diaphoresis, or hemodynamic instability, and the extent of disease ranges from single-vessel to multivessel involvement. Diagnosis integrates the clinical history, serial electrocardiography, biomarker measurement, and imaging including echocardiography to assess regional wall motion and ventricular function, while risk stratification guides the urgency and intensity of treatment. Management combines antiplatelet and anticoagulant therapy, anti-ischemic medication, and, where indicated, prompt reperfusion through percutaneous coronary intervention or, in selected cases, surgical revascularization. Because ACS arises from modifiable cardiovascular risk factors, prevention through control of hypertension, dyslipidemia, diabetes, and lifestyle is integral to reducing incidence and recurrence. Research addresses pathophysiology, biomarkers, risk prediction, and therapeutic strategies aimed at limiting infarct size and improving outcomes.
