Overview

Esophageal cancer is a malignancy that develops in the cells lining the esophagus, the muscular tube that carries food and liquid from the throat to the stomach. Its two principal histological types are squamous cell carcinoma and adenocarcinoma, the latter often associated with chronic acid reflux and Barrett's esophagus. It is a serious disease, frequently diagnosed at an advanced stage because early symptoms are subtle, and difficulty swallowing, weight loss, and pain are common as it progresses. Recognised risk factors include tobacco and alcohol use, obesity and reflux, dietary factors, and certain infections and predisposing conditions. Research relevant to esophageal cancer includes the study of molecular markers such as microRNA expression, where high tissue expression of miR-21 has been correlated with poorer patient survival, and the role of human papillomavirus in virus-induced carcinogenesis. Because the esophagus is central to swallowing and nutrition, the disease and its treatment have major nutritional consequences, and the management of feeding following esophagectomy is an important aspect of care. Treatment may combine surgery, chemotherapy, radiation, and supportive measures, guided by tumour type, stage, and patient status. Key themes include early detection, risk-factor reduction, molecular and prognostic markers, and the nutritional impact of disease and surgery, which together inform diagnosis, prognosis, and the comprehensive management of affected patients.
