Overview
Corneal disorders are diseases, dystrophies, and injuries affecting the cornea, the transparent, avascular, dome-shaped tissue forming the eye's anterior surface and providing most of its refractive power. Because corneal clarity and regular curvature are essential for focusing light onto the retina, these conditions can degrade vision through opacity, irregular astigmatism, edema, or scarring. They are commonly grouped by mechanism and layer involved: infectious keratitis from bacterial, viral, fungal, or protozoal pathogens; inflammatory and immune-mediated disease such as peripheral ulcerative keratitis and Mooren's ulcer; ectatic disorders including keratoconus, in which progressive stromal thinning produces corneal protrusion; inherited corneal dystrophies affecting the epithelium, stroma, or endothelium; degenerations; and traumatic or chemical injury. Endothelial dysfunction impairs the pump function that maintains stromal deturgescence, leading to corneal edema. Diagnosis relies on slit-lamp biomicroscopy, corneal topography and tomography, pachymetry, specular and confocal microscopy, and staining. Management spans medical therapy, contact lenses, and surgical intervention, ranging from corneal collagen cross-linking to arrest keratoconus, to lamellar and penetrating keratoplasty for advanced opacity or endothelial failure. Untreated, severe corneal disease is a major cause of preventable visual impairment, making timely recognition and treatment central to ophthalmic care.
