Overview

Weight loss is a reduction in total body mass, which may be intentional, pursued to improve health and reduce the risks associated with overweight and obesity, or unintentional, signalling underlying disease. Deliberate weight management aims to lower fat mass while preserving lean tissue and is achieved through negative energy balance using dietary modification, increased physical activity, behavioural change, pharmacotherapy, and, for severe obesity, bariatric and endoscopic procedures. Sustained loss reduces the burden of type 2 diabetes, hypertension, dyslipidaemia, and certain cancers, but maintenance is challenged by adaptive physiological responses that defend body weight. Research in this area examines predictive markers and modelling of weight loss following Roux-en-Y gastric bypass, outcomes of sequential intragastric balloon treatment in super obesity, and high-protein supplementation in obese adolescents. Dietary approaches are explored through food energy density and weight maintenance in older patients, daily grape-juice consumption, and the efficacy of a hypocaloric Mediterranean diet, while mechanistic work addresses adipose-tissue physiology, body-composition modelling, and the effects of fennel, alone and combined with atorvastatin, in animal models of obesity. Together these reflect interest in both the clinical management and underlying biology of weight reduction.
