Overview
A cerebrovascular accident, commonly termed a stroke, is an acute neurological injury caused by interruption of blood supply to part of the brain, resulting in focal loss of function. It is classified as ischemic, when a thrombus or embolus occludes a cerebral artery and deprives tissue of oxygen and glucose, or hemorrhagic, when a vessel ruptures and bleeds into or around brain tissue, with transient ischemic attacks representing reversible episodes. Clinical consequences depend on the territory and extent of injury and may include hemiparesis, aphasia, visual loss, and cognitive impairment, while outcome is shaped by the speed of restoration of perfusion and by secondary processes including oxidative injury and edema. Risk factors overlap with those for cardiovascular disease, notably hypertension, atrial fibrillation, diabetes, and atherosclerosis, and assessment of cerebral hemodynamics, endothelial function, and vascular reactivity informs understanding of subtype and prognosis.
