Overview

Cerebrovascular disease encompasses a group of conditions that impair blood flow to the brain through narrowing, occlusion, rupture, or malformation of the cerebral vasculature, producing ischaemic or haemorrhagic injury to neural tissue. Its principal manifestations include ischaemic and haemorrhagic stroke, transient ischaemic attack, cerebral infarction, and vascular cognitive impairment, with atherosclerosis, thromboembolism, small-vessel disease, and endothelial dysfunction as core pathological mechanisms. Established risk factors include hypertension, diabetes mellitus, dyslipidaemia, smoking, atrial fibrillation, and hyperhomocysteinaemia, several of which converge on damage to the endothelium and the arterial wall. Coronavirus infection has been linked to thrombotic and cerebrovascular complications through endothelial injury, coagulopathy, and systemic inflammation, situating the topic within both vascular neurology and infectious disease. Articles associated with this topic across the collection address cerebrovascular infarction presenting as internuclear ophthalmoplegia, endothelial function across stroke subtypes, homocysteine and carotid intima-media thickness, ACE gene polymorphism in hypertension and stroke, and inflammatory and prognostic markers in COVID-19 pneumonia, reflecting peer-reviewed research relevant to cerebrovascular mechanisms and risk. Characterising cerebrovascular disease and its determinants is important because stroke is a leading cause of disability and death, and identification of modifiable vascular risk and endothelial pathophysiology underpins prevention and acute management.
