Overview
Acute respiratory distress syndrome (ARDS) is a severe, life-threatening form of respiratory failure marked by rapid onset of widespread inflammation in the lungs, leading to fluid accumulation in the alveoli, impaired oxygen exchange, and profound hypoxemia. It can be triggered by infections, including viral pneumonia, and became a prominent complication of severe COVID-19, often requiring oxygen support or mechanical ventilation. Within this journal, ARDS is examined largely in the context of coronavirus infection and severe respiratory illness. Diagnostic research includes whether the alveolar-arterial oxygen pressure difference can help diagnose ARDS in pneumonia patients. Studies of severe COVID-19 that bear on the pathophysiology of respiratory distress include cytokine profiling in patients during the pre-storm phase, the effect of SARS-CoV-2 spike protein on interferon and cytokine gene expression, and the evaluation of mortality-risk scoring tools and prognostic markers in COVID-19 pneumonia. Imaging research on the role of chest computed tomography supports assessment of lung involvement, while work on cell therapy considers approaches to the consequences of infection. Related studies address treatment evaluation, such as hydroxychloroquine, and the broader clinical spectrum of coronavirus disease, including neurological and extrapulmonary complications. Together these aspects reflect the diagnosis, underlying inflammatory mechanisms, and management of acute respiratory distress in the setting of severe respiratory infection.
