Overview
Vascular disease encompasses disorders of the blood vessels, including the arteries, veins, lymphatics and microvasculature, that disrupt the delivery of blood to tissues and the return of blood to the heart. Arterial disorders include atherosclerosis, peripheral artery disease, aneurysm and arterial occlusion; venous disorders include thrombosis and chronic venous insufficiency; and microvascular disease underlies many of the complications of diabetes and hypertension. A central mechanism across these conditions is endothelial dysfunction, in which the vessel lining loses its capacity to regulate vasodilation, thrombosis and inflammation, promoting ischemia, tissue injury and impaired wound healing. Vascular disease is closely linked to systemic risk factors and frequently coexists with renal and cardiac disease. Studies in this area examine cardiovascular disease in patients undergoing dialysis, aneurysm of the common iliac artery, the vascular and systemic toxicity of contrast agents, diabetic foot and leg ulceration with peri-wound neuropathy, endothelial function across stroke subtypes, and microalbuminuria as a marker of microvascular and endothelial injury. By relating vessel structure and function to clinical outcome, this field clarifies how arterial, venous and microvascular pathology contribute to ischemia, organ damage and the broader burden of cardiovascular disease.
