Overview

Heart failure is a clinical syndrome in which the heart is unable to pump blood efficiently enough to meet the body's needs, either because the heart muscle is weakened and cannot contract forcefully or because it has become stiff and cannot fill properly. Also called cardiac failure or congestive heart failure, it can affect the left or right side of the heart and may develop gradually or appear acutely. Common features include breathlessness, fatigue, and fluid retention, and the condition can result from many underlying problems such as coronary artery disease, hypertension, valve disorders, and other cardiac or systemic diseases. Heart failure is a major cause of illness and death worldwide and often coexists with disorders of other organ systems, making its assessment and management complex. The research presented in this journal reflects several of these dimensions, including cardiorenal signalling pathways that link the heart and kidneys, assessment of cardiac function and sleep-disordered breathing, the management of heart failure in primary care, and emerging therapeutic strategies such as gene therapy and microRNA-based approaches for ischaemic heart disease. Related articles address rheumatic and valvular heart disease, hypertensive crisis, cardiac mechanics in hypertension, and cardiovascular complications in patients on dialysis, illustrating the diverse causes, manifestations, and evolving treatments associated with heart failure.
