Overview
Community healthcare is the delivery of health services organized around a defined population or geographic area, emphasizing the promotion of health, prevention of disease, and equitable access to care close to where people live. It extends beyond treatment in clinical facilities to encompass health promotion, screening, management of chronic and communicable disease, maternal and child services, and the social determinants that shape health within a community. Community healthcare frequently relies on community health workers, primary-care facilities, and collaborative health systems that link populations to higher levels of care, and it aims to improve population health outcomes while reducing disparities. Its effectiveness depends on appropriate models of care delivery, the engagement of local providers and managers, the quality of facilities, and the responsiveness of services to the needs of vulnerable groups.
