Overview
Renal cancer encompasses malignancies arising in the kidney, the most common being renal cell carcinoma, which originates in the epithelial lining of the renal tubules. Clear cell renal cell carcinoma is the predominant histological subtype and is frequently associated with inactivation of the VHL tumor-suppressor gene, leading to dysregulated hypoxia-inducible factor signaling and abnormal angiogenesis; papillary and chromophobe carcinomas represent additional distinct subtypes with their own molecular features. Tumors may grow silently within the renal parenchyma and are often discovered incidentally on imaging performed for unrelated reasons, although hematuria, flank pain, or a palpable mass can occur in more advanced disease. Staging integrates tumor size, local invasion, and the presence of nodal or distant metastasis, guiding both prognosis and treatment. Surgical resection, through partial or radical nephrectomy, remains the principal curative approach for localized tumors. Advanced and metastatic disease is managed with agents targeting the vascular endothelial growth factor pathway and the mechanistic target of rapamycin, as well as immune checkpoint inhibitors that restore antitumor immunity. Ongoing research addresses molecular classification, resistance mechanisms, and predictive biomarkers to refine therapy selection and improve survival across the heterogeneous spectrum of renal malignancy.
