Overview

Ulcerative colitis is a chronic inflammatory bowel disease characterized by continuous inflammation and ulceration of the innermost lining (mucosa) of the colon and rectum. It typically follows a relapsing and remitting course and commonly presents with bloody diarrhea, abdominal pain and cramping, urgency, and, over time, weight loss and fatigue. The precise cause is not fully understood, but ulcerative colitis is thought to arise from a dysregulated immune response to intestinal contents in genetically susceptible individuals, influenced by environmental factors and the gut microbiome. Unlike Crohn's disease, which can affect any part of the gastrointestinal tract in a patchy fashion, ulcerative colitis is confined to the colon and progresses continuously from the rectum. Diagnosis relies on endoscopy with biopsy, supported by imaging and laboratory markers, while management ranges from aminosalicylates, corticosteroids, and immunomodulators to biologic therapies and, in refractory or complicated cases, surgery. Long-standing disease also carries an increased risk of colorectal cancer, making surveillance important.
