Overview

Anxiety disorders are a group of mental health conditions defined by excessive, persistent fear or anxiety and related behavioural disturbances that are disproportionate to actual threat and cause clinically significant distress or functional impairment. The category includes generalized anxiety disorder, panic disorder, social anxiety disorder, specific phobias, agoraphobia, and separation anxiety, distinguished by the focus and pattern of the fear response. Their pathophysiology implicates dysregulated threat-detection circuitry centred on the amygdala and its prefrontal and hippocampal connections, altered serotonergic, GABAergic, and noradrenergic signalling, neuropeptide systems such as cholecystokinin, and autonomic and hypothalamic-pituitary-adrenal reactivity, shaped by genetic vulnerability and environmental stress. They commonly coexist with depressive, eating, and neurodevelopmental disorders, complicating diagnosis and outcome. Diagnosis follows standardized clinical criteria and structured measures, and evidence-based treatment includes cognitive-behavioural therapy, exposure-based methods, pharmacotherapy, and adjuncts such as biofeedback and heart-rate-variability training.
