Overview

Colorectal cancer screening is the systematic testing of asymptomatic individuals to detect colorectal cancer and its precursor lesions at an early, more treatable stage, with the dual goals of reducing incidence through removal of premalignant polyps and lowering mortality through earlier diagnosis. Established modalities include stool-based tests such as the fecal immunochemical test, endoscopic examinations including colonoscopy and sigmoidoscopy, and structured population programs that determine eligibility, intervals, and follow-up. Research in this area examines both the performance of screening tests and the design and implementation of screening programs. Studies of company-based and population-based screening evaluate compliance and yield using fecal immunochemical testing across large cohorts, and consider how programs can be developed in varied health-system settings. Investigations of proximal colonic anatomy and serrated lesions address the biological basis of interval cancers that escape detection, while work on emerging molecular approaches, including microRNA quantification in stool, points toward complementary screening tools.
