Overview

Colorectal cancer prognosis concerns the prediction of disease course and outcome in cancers of the colon and rectum, and the factors that determine survival after diagnosis. Prognosis depends heavily on stage at presentation, defined by depth of tumor invasion, lymph node involvement, and the presence of metastasis, with early-stage disease carrying a far better outlook than advanced disease. Beyond stage, tumor biology shapes prognosis: histologic grade, molecular and genetic alterations, and the expression of adhesion and signaling molecules such as E-cadherin and vascular cell adhesion molecules influence the propensity for invasion and metastasis. Research seeks prognostic and predictive markers, including gene-expression changes in tumor tissue, microRNA signatures, and proteins that regulate apoptosis and metastatic spread, to refine risk assessment and guide treatment. Clinical factors also matter; perioperative cardiac risk markers, for instance, affect surgical outcomes in patients undergoing resection. Early detection through screening, including fecal immunochemical testing and colonoscopy, improves prognosis by identifying disease at a more treatable stage and by removing precancerous serrated and adenomatous lesions before malignancy develops. Prognostication integrates pathological staging, molecular characterization, biomarker analysis, and patient factors to estimate likely outcomes and to individualize therapy, supporting decisions that aim to improve survival and quality of life in colorectal cancer.
