Overview

Cognitive impairment is a measurable decline in one or more domains of mental function, including memory, attention, executive function, language, and processing speed, that exceeds expected age-related change. It spans a continuum from mild cognitive impairment, in which deficits are detectable but daily independence is largely preserved, to dementia, in which functional decline is substantial. Causes are diverse and include neurodegenerative disease such as Alzheimer's disease, cerebrovascular injury and stroke, metabolic and endocrine disturbance, and the cognitive effects of depression and chronic stress; depression itself is both a risk factor for and a frequent accompaniment of cognitive decline. Assessment relies on neuropsychological testing, neuroimaging of structures such as the medial temporal lobe, and biomarkers including cortisol. Research relevant to this topic includes cognitive stimulation training and the factors that sustain participation, basal cortisol and medial temporal lobe atrophy in mild cognitive impairment and Alzheimer's disease, early stressful life events as antecedents of dementia, long-term memory in relation to mental activity and type 2 diabetes, the relationship between depression and dementia, and the cognitive sequelae of COVID-19. Methods include qualitative, observational, and clinical designs. The journal publishes peer-reviewed work on the detection, determinants, and management of cognitive impairment.
