Overview

Osteoporosis is a systemic skeletal disorder characterised by low bone mass and microarchitectural deterioration, leading to fragile bones and a heightened risk of fractures, especially of the hip, spine, and wrist, and it is among the most common degenerative conditions of ageing. It develops when osteoclastic bone resorption exceeds osteoblastic formation during continuous remodelling, a balance disturbed by advancing age, the postmenopausal decline in oestrogen, calcium and vitamin D insufficiency, reduced physical activity, and secondary disease. Because bone loss progresses silently until fracture occurs, diagnosis depends on measurement of bone mineral density and assessment of trabecular structure, together with evaluation of risk factors and falls. In older adults the consequences of fracture, including loss of independence, disability, and secondary fractures, make prevention a priority. Management combines adequate calcium and vitamin D, weight-bearing activity, fall prevention, and pharmacological agents that suppress resorption or stimulate formation. The peer-reviewed research collected under this topic addresses osteoporosis knowledge and calcium intake in women, vitamin D and balance in older adults, secondary hip-fracture risk, trabecular bone structure, sleep disturbance and hip fracture, and experimental models of bone loss, reflecting both the clinical management of skeletal fragility and the mechanisms underlying the prevention and treatment of osteoporosis in ageing.
