Overview

Elder care is the organized provision of health, personal, and social support to older adults whose ageing-related physical, cognitive, or functional changes create needs for assistance. It spans a continuum of settings, from independent living with community support to home-based care, assisted living, long-term care facilities, and specialized geriatric services, and integrates medical management with help in activities of daily living. Effective elder care addresses the complexity of older patients, who frequently present with multiple chronic conditions, polypharmacy, sensory impairment, and heightened vulnerability to frailty, falls, and cognitive decline. A central principle is person-centered care, which aligns services with the individual's values, preferences, and goals rather than disease alone, and emphasizes dignity, autonomy, and continuity across providers. Care models also attend to mental health, addressing depression, anxiety, loneliness, and social isolation that accompany later life, and to caregiver support, since family members often share responsibility for medication administration and daily supervision. Coordination across primary care, specialists, rehabilitation, and social services is essential to managing transitions and preventing avoidable hospitalization. Research in elder care evaluates interventions that promote healthy ageing, optimize function, and improve quality of life, including health literacy and prevention programs. By combining clinical, psychological, and social dimensions, elder care seeks to sustain wellbeing and independence for ageing populations.
